I saw this at my doctor’s and i thought people in london don’t follow this simple advise often enough.. This book is over 120 pages long; however, 60 pages contain forms that the reader could fill with certain details on a daily bases to keep a diary. There are 5 different forms repeated 12 times to cover 12 weeks. If mr.
